***All new players must bring a Birth Certificate in order to register for the 2012
season***
Registration Payment Dates - We will send out
an email the week of the payment date.
Tuesday,
February 14th –
6:00pm to 8:00pm
In-person payments will take place at:
Hamilton B. Maher
Community Center at 3450 W. 97th Street
If you do not register online and pay by 2/14/12, you will be subject to
the late fees.
Registration online is fast and easy.
1. Fill out the form below, print the
confirmation page (which includes your $10 off
coupon)
2. Bring your printed confirmation page to
one of the registration dates above with your
checkbook.

**In accordance with
the Babe Ruth Baseball
guidelines, the age
determination date is
April 30th. Whatever
the player’s age is on
April 30, 2012 will be
the player's
league age for
the 2012 season.
Payment must be made by two
(2) checks payable to the
Evergreen Park Boys Club (“EPBC”).
One check will cover the Fee
and the Raffle Book and the
second check in the amount of
$150.00 will be held for the
Volunteer Incentive Program.
At registration, parents will
need the following:
- Birth Certificate - for new players
(must be 5 years
old by 4/30/12 to play)
- One check for: Division Fee(s) + $50 Raffle
Book (1 per family).
- Second Check for:
$150 Volunteer Deposit
(per family). When parents successfully
complete the Incentive Program, they will receive
their un-cashed check back.
- Confirmation Form: after you fill out the form below (one per child),
click submit, print the confirmation page and bring
it to a registration payment date.
Safety Regulations: All
players MUST wear an
Athletic Supporter AND a
Heart Guard. (Items may be
purchased at most sporting
goods stores.)
