Our payment dates for registration are TBD for the 2012 season.  You may now register below.  Please see the table below for applicable fees.

***All new players must bring a Birth Certificate in order to register for the 2012 season***

Registration Payment Dates - We will send out an email the week of the payment date.

Tuesday, February 14th – 6:00pm to 8:00pm

In-person payments will take place at:
Hamilton B. Maher Community Center at 3450 W. 97th Street
If you do not register online and pay by 2/14/12, you will be subject to the late fees.

Registration online is fast and easy.

1. Fill out the form below, print the confirmation page (which includes your $10 off coupon)
2. Bring your printed confirmation page to one of the registration dates above with your checkbook.

**In accordance with the Babe Ruth Baseball guidelines, the age determination date is April 30th.  Whatever the player’s age is on April 30, 2012 will be the player's league age for the 2012 season.

Payment must be made by two (2) checks payable to the Evergreen Park Boys Club (“EPBC”).  One check will cover the Fee and the Raffle Book and the second check in the amount of $150.00 will be held for the Volunteer Incentive Program. 

At registration, parents will need the following:

1. Birth Certificate - for new players (must be 5 years old by 4/30/12 to play)
2. One check for: Division Fee(s) + $50 Raffle Book (1 per family).
3. Second Check for: $150 Volunteer Deposit (per family).  When parents successfully complete the Incentive Program, they will receive their un-cashed check back.
4. Confirmation Form: after you fill out the form below (one per child), click submit, print the confirmation page and bring it to a registration payment date.

Safety Regulations:  All players MUST wear an Athletic Supporter AND a Heart Guard.  (Items may be purchased at most sporting goods stores.)

Volunteer Information
Each family is required to volunteer their time in some way as needed to run this league. I understand that I am required to work a Volunteer Job in order to receive a refund of my Volunteer Fee as part of the Volunteer Incentive Program. If I choose not to volunteer, I hereby agree to surrender my Volunteer Fee to the Evergreen Park Boys Club. If I am unable to complete my assigned Volunteer Job, it is my responsibility to contact the league to be reassigned a different Volunteer Job. Additionally, if I sign up for a Manager’s position, I understand that I must also sign up for another Volunteer Job as I am not guaranteed this position. (Please Rank in order of preference from 1 to 6, with 1 being you’re most desired choice.)

Find a team Sponsor and your volunteer duty is waived! In order to maintain fees and make capital field improvements, we are actively seeking a corporate sponsor for each team in the league this year.  If you know of and can solicit a company for a full $350 team sponsorship, we will waive your volunteer duty.  You must secure and deliver the sponsorship fee to us by 12/31/2011 in order to qualify.  You may use this Team Sponsorship Letter when soliciting companies.  Please email us if you have any questions

I/We, the parent(s) or legal guardian(s) of the named candidate for a position on a league team, hereby give my/our approval to participate in any and all league activities. I also grant permission to league supervisors to authorize and obtain medical care from any licensed physician, hospital or medical clinic should the player become ill or injured while participating in league activities away from home, or at other times when neither parent is available to grant authorization for emergency treatment.
